You may refer patients to our office by filling out our secure online Referral Form. After you have completed the form, please make sure to press the Complete and Send button at the bottom to automatically send us your information. The security and privacy of patient data is one of our primary concerns and we have taken every precaution to protect it.
Insurance plans we accept
Your patients can get oral surgery care close to home, with their benefits. Lake County Oral Surgery is in-network with Aetna, Ameritas, Assurant, Blue Cross Blue Shield of Illinois, Delta Dental Premier, DNOA PPO, GEHA, Guardian, Lincoln Financial Group, United Concordia, Sunlife, Humana, MetLife PDP Plus, Principal Financial Group, and UnitedHealthcare. We also accept the Careington discount plan.
What your office and your patients can expect:
- Benefits verified within one business day of your referral
- A call to your patient to schedule within one business day
- A consultation available within seven days
- A consult note back to you within 48 hours, and a post-op note after surgery
